Intake Forms
The following forms are designed to help us more easily work together: Please print and complete all 4 forms prior to therapist arrival. Your therapist will read over the forms and add information based questions asked during the intake process. Your therapist will sign the forms and provide you an opportunity to photograph the forms for your records.
